Reablement Support Worker
Key Responsibilities
  • To undertake the role of a Reablement Support Worker, for the Home First Plus (home from hospital reablement service), you will be providing on call cover for HCL at all times for sickness/absence/holidays of Community Social Care workers, across a range of localities and to provide short term care for customers discharged from hospital at short notice. The post holder will also support the delivery of care staff development by supporting on the job training, coaching and support in the workplace.
Main Duties
  • Provide on call support to cover periods of planned and unplanned absence including sickness, absence & holidays of care workers across a range of localities.
  • Provide home from hospital support to customers being discharged from hospital at short notice, ensuring they are safe and supported to reduce the risks of readmission into hospital.
  • Support the development of the Community Social Care workforce through delivery of on the job training, coaching and support.
  • Promote choice, involvement & independence of customers by supporting them to make their own decisions regarding their care and to meet their own goals and aspirations.
  • Shadow new Community Social Care workers, identifying individual training needs, providing coaching/mentoring support as appropriate.
  • Ensure all shadowing documentation is completed and submitted to the Locality Manager, reporting to them without delay any areas of concern relating to practice which need additional support/action.
  • Ensure customer care is delivered with dignity and respect and in accordance with the standards laid down within the Fundamental Standards of Care.
  • Ensure customers and their families are enabled to access other services to meet their needs, including signposting to additional services as appropriate.
  • Develop a relationship with the customer their carer, and family to actively engage them in the care planning process, working in partnership with them to achieve person-centred care is delivered which meets their needs and personal goals.
  • Assist customers with personal care needs where necessary and appropriate for example getting up in the morning washing, dressing and toileting.
  • Promote reablement by encouraging customers to work in partnership with you to follow an agreed, personalised care plan.
  • Prepare meals/snacks where necessary promoting good nutrition and hydration, ensuring good practice in food preparation and hygiene.
  • Administer Medication to customers where required adhering HCL’s Medicines Policy and procedures.
  • Respond to requests from customers, their carer’s, families or directions from the Locality Manager in order to facilitate contact outside the home for example attendance at day centres and other social activities.
  • Signpost customers, their carer’s and family to other support services for example Age Concern and the Live Well Directory livewell.co.uk
  • Contribute to the ‘One Liverpool’ plan by adopting and promoting HCL’s commitment to health and wellbeing of both customers and staff.
  • Update the Locality Manager about the wellbeing of customer and the progress of care provided, reporting any issues or concerns without delay.
  • Comply with HCL policy on attendance at a customer’s residence using the electronic app PASS.
  • Keep accurate and compliant records by recording all visits in the Customers Care Plan via the electronic app PASS.
  • Attend staff meetings as requested or required.
  • Attend and contribute to planned supervision sessions with the Locality Manager.
  • Through training and information sharing keep up to date with any changes in practice affecting social care.
  • Ensure compliance with HCL’s Equality and Diversity Policy.
  • Successfully complete mandatory training within acceptable time frames, including the nationally recognised Care Certificate if appropriate.
  • Ensure that the service you provide respects the individual customer as an adult and equal citizen.
  • Ensure the services you deliver promote the physical, emotional, spiritual and cultural needs of the service users to support customers to express their individuality and uniqueness in all areas of their lives and that any cultural needs are recognised, assessed and met where possible.
  • Maintain a good working relationship with other colleagues to ensure that effective working and communication is maintained.
  • Participate in shadowing new Community Social Care Staff, as required or requested to do so, considering the level of experience, skills and knowledge of the post holder.
  • Ensure you adhere to all HCL policies and procedures as set out within the staff handbook, paying regard Health & Safety at Work Act 1974, Equality and Diversity and Data Protection Act 2018 (GDPR).
  • Ensure compliance with the UKHCA codes of practice and rules and HCL policies and procedures contained in the Staff Handbook and HCL Code of Conduct.
  • Undertake mandatory training as required and maintain CPD as identified and agreed with the Locality Manager as part of supervision and/or as part of an annual appraisal of performance.
  • Act as an ambassador for HCL when representing the Company while undertaking your role.
  • Ensure that the values and ethos of HCL are always upheld.
  • Participate in HCL social and fundraising activities as required, including celebrating success of both staff, customers, colleagues and partners.
Required Education, Skills and Qualifications - Essential
  • Available to work 7am-2pm & 4pm-11pm.
  • Driver with access to own vehicle.
  • Diploma/N.V.Q. 2 or equivalent in Health and Social Care minimum or willing to enrol onto Diploma
  • Previous experience of working with vulnerable adults.
  • Caring and empathetic with a focus on the delivery of quality and compassionate care services
  • Literacy and Numeracy-must be able to complete accurate and compliant records in relation to care delivery and medication administration via a mobile phone app.
  • Good communicator with the ability to encourage service users and engage with family members.
  • To be flexible at all times to cover sickness/absence/holidays of care workers across a range of localities.
  • Self- motivated with an ability to work independently or as part of a team
