Assist customers with personal care needs where necessary and appropriate for example getting up in the morning washing, dressing and toileting.
Promote reablement by encouraging customers to work in partnership with you to follow an agreed, personalised care plan.
Prepare meals/snacks where necessary promoting good nutrition and hydration, ensuring good practice in food preparation and hygiene.
Administer Medication to customers where required adhering HCL’s Medicines Policy and procedures.
Respond to requests from customers, their carer’s, families or directions from the Locality Manager in order to facilitate contact outside the home for example attendance at day centres and other social activities.
Signpost customers, their carer’s and family to other support services for example Age Concern and the Live Well Directory livewell.co.uk
Contribute to the ‘One Liverpool’ plan by adopting and promoting HCL’s commitment to health and wellbeing of both customers and staff.
Update the Locality Manager about the wellbeing of customer and the progress of care provided, reporting any issues or concerns without delay.
Comply with HCL policy on attendance at a customer’s residence using the electronic app PASS.
Keep accurate and compliant records by recording all visits in the Customers Care Plan via the electronic app PASS.
Attend staff meetings as requested or required.
Attend and contribute to planned supervision sessions with the Locality Manager.

Ensure compliance with HCL’s Equality and Diversity Policy.
Successfully complete mandatory training within acceptable time frames, including the nationally recognised Care Certificate if appropriate.

Maintain a good working relationship with other colleagues to ensure that effective working and communication is maintained.
Participate in shadowing new Community Social Care Staff, as required or requested to do so, taking into account the level of experience, skills and knowledge of the post holder.

Ensure compliance with the UKHCA codes of practice and rules and HCL policies and procedures contained in the Staff Handbook and HCL Code of Conduct.
Undertake mandatory training as required and maintain CPD as identified and agreed with the Locality Manager as part of supervision and/or as part of an annual appraisal of performance.
Act as an ambassador for HCL when representing the Company while undertaking your role.
Ensure that the values and ethos of HCL are always upheld.
Participate in HCL social and fundraising activities as required, including celebrating success of both staff, customers, colleagues and partners.
